Student Work Permit Request

 

All of the following areas need to be completed before returning to us. Scan or take a picture of the completed form and email it back to the following email address: [email protected]

 

These are the areas that need to be filled out:

  1. MINOR’S Information
  2. SCHOOL Information
  3. To be filled in and signed by PARENT OR LEGAL GUARDIAN
  4. To be filled in and signed by EMPLOYER
 

When we receive the form back, the process will take one to two business days. Someone in the office will give you a call when the work permit is done.  Students must come to the office to pick up the completed form and sign it.
